Make a Referral

If you're supporting someone who needs a safe, stable place to rebuild, this form starts the conversation. You don't need every answer — tell us what you know, and we'll take it from there.

Before you start

Please only submit a referral if the person has agreed to it. If you are unsure about an answer, write what you know — an incomplete referral is better than none.

The form takes about ten minutes and saves as you go, so you can come back to it.
We aim to respond within two working days.
If someone is at immediate risk, please call 01803 446133 rather than waiting for a reply.
